Introduction:
Engaging patients, caregivers, and other stakeholders to help guide the research process is a cornerstone of patient-centered research. Lived expertise may help ensure the relevance of research questions, promote practices that are satisfactory to research participants, improve transparency, and assist with disseminating findings.
Methods:Traditionally engagement has been conducted face-to-face in the local communities in which research operates. Decentralized platform trials pose new challenges for the practice of engagement. We used a remote model for stakeholder engagement, relying on Zoom meetings and blog communications.
Results:Here we describe the approach used for research partnership with patients, caregivers, and clinicians in the planning and oversight of the ACTIV-6 trial and the impact of this work. We also present suggestions for future remote engagement.
Conclusions:The ACTIV-6 experience may inform proposed strategies for future engagement in decentralized trials.
The competency-based medical education evolution of Canadian emergency medicine specialist training

Canadian specialist emergency medicine (EM) residency training is undergoing the most significant transformation in its history. This article describes the rationale, process, and redesign of EM competency-based medical education. The rationale for this evolution in residency education includes 1) improved public trust by increasing transparency of the quality and rigour of residency education, 2) improved fiscal accountability to government and institutions regarding specialist EM training, 3) improved assessment systems to replace poor functioning end-of-rotation assessment reports and overemphasis on high-stakes, end-of-training examinations, and 4) and tailored learning for residents to address individualized needs. A working group with geographic and stakeholder representation convened over a 2-year period. A consensus process for decision-making was used. Four key design features of the new residency education design include 1) specialty EM-specific outcomes to be achieved in residency; 2) designation of four progressive stages of training, linked to required learning experiences and entrustable professional activities to be achieved at each stage; 3) tailored learning that provides residency programs and learner flexibility to adapt to local resources and learner needs; and 4) programmatic assessment that emphasizes systematic, longitudinal assessments from multiple sources, and sampling sentinel abilities. Required future study includes a program evaluation of this complex education intervention to ensure that intended outcomes are achieved and unintended outcomes are identified.

The importance of parasites as a selective force in host evolution is a topic of current interest. However, short-term ecological studies of host–parasite systems, on which such studies are usually based, provide only snap-shots of what may be dynamic systems. We report here on four surveys, carried out over a period of 12 years, of helminths of spiny mice (Acomys dimidiatus), the numerically dominant rodents inhabiting dry montane wadis in the Sinai Peninsula. With host age (age-dependent effects on prevalence and abundance were prominent) and sex (female bias in abundance in helminth diversity and in several taxa including Cestoda) taken into consideration, we focus on the relative importance of temporal and spatial effects on helminth infracommunities. We show that site of capture is the major determinant of prevalence and abundance of species (and higher taxa) contributing to helminth community structure, the only exceptions being Streptopharaus spp. and Dentostomella kuntzi. We provide evidence that most (notably the Spiruroidea, Protospirura muricola, Mastophorus muris and Gongylonema aegypti, but with exceptions among the Oxyuroidae, e.g. Syphacia minuta), show elements of temporal-site stability, with a rank order of measures among sites remaining similar over successive surveys. Hence, there are some elements of predictability in these systems.

Objective
Policy-makers and practitioners have a need to assess community resilience in disasters. Prior efforts conflated resilience with community functioning, combined resistance and recovery (the components of resilience), and relied on a static model for what is inherently a dynamic process. We sought to develop linked conceptual and computational models of community functioning and resilience after a disaster.
MethodsWe developed a system dynamics computational model that predicts community functioning after a disaster. The computational model outputted the time course of community functioning before, during, and after a disaster, which was used to calculate resistance, recovery, and resilience for all US counties.
ResultsThe conceptual model explicitly separated resilience from community functioning and identified all key components for each, which were translated into a system dynamics computational model with connections and feedbacks. The components were represented by publicly available measures at the county level. Baseline community functioning, resistance, recovery, and resilience evidenced a range of values and geographic clustering, consistent with hypotheses based on the disaster literature.
ConclusionsThe work is transparent, motivates ongoing refinements, and identifies areas for improved measurements. After validation, such a model can be used to identify effective investments to enhance community resilience. (Disaster Med Public Health Preparedness. 2018;12:127–137)

Objective
We sought to conduct a major objective of the CAEP Academic Section, an environmental scan of the academic emergency medicine programs across the 17 Canadian medical schools.
MethodsWe developed an 84-question questionnaire, which was distributed to academic heads. The responses were validated by phone by the lead author to ensure that the questions were answered completely and consistently. Details of pediatric emergency medicine units were excluded from the scan.
ResultsAt eight of 17 universities, emergency medicine has full departmental status and at two it has no official academic status. Canadian academic emergency medicine is practiced at 46 major teaching hospitals and 13 specialized pediatric hospitals. Another 69 Canadian hospital EDs regularly take clinical clerks and emergency medicine residents. There are 31 full professors of emergency medicine in Canada. Teaching programs are strong with clerkships offered at 16/17 universities, CCFP(EM) programs at 17/17, and RCPSC residency programs at 14/17. Fourteen sites have at least one physician with a Master’s degree in education. There are 55 clinical researchers with salary support at 13 universities. Sixteen sites have published peer-reviewed papers in the past five years, ranging from four to 235 per site. Annual budgets range from $200,000 to $5,900,000.
ConclusionThis comprehensive review of academic activities in emergency medicine across Canada identifies areas of strengths as well as opportunities for improvement. CAEP and the Academic Section hope we can ultimately improve ED patient care by sharing best academic practices and becoming better teachers, educators, and researchers.

Summary
Abstract Over the past century, the Santa Cruz Formation of coastal Argentina (late Early Miocene) has yielded a remarkable collection of platyrrhine primates. With few notable exceptions, most of the specimens have been included in
Ameghino, 1891, a stem platyrrhineHomunculus patagonicus . Homunculus patagonicus was approximately 1.5 to 2.5 kg in body mass, about the size of a living saki monkey (Pithecia ) or a femaleCebus . Molar structure indicates that the diet consisted of a mixture of fruit and leaves. A deep jaw, large postcanine tooth roots, large postglenoid processes and moderately large chewing muscle attachments (i.e. massive zygomatic arches, sculpted temporalis origins) suggest that physically resistant foods were key components of the diet. Heavy tooth wear suggests large amounts of ingested silica or exogenous abrasives. Incisor morphology suggests that exudate harvesting may have been part of the behavioral repertoire, although not a specialization. The canines were small, providing no evidence of sclerocarpic foraging. Canines were sexually dimorphic, suggesting that the taxon experienced some intrasexual competition rather than being solitary or pair-bonded. Brain size was small and the frontal cortical region was proportionately small. From the small size and structure of the orbits, the structure of the organ of hearing, the reduced olfactory fossae and the relatively large infraorbital foramina, we infer thatHomunculus was probably diurnal, with acute vision and hearing, but with a poor sense of smell and little reliance on tactile vibrissae.Homunculus was an above-branch arboreal quadruped with leaping abilities. The semicircular canals show evidence of considerable agility, reinforcing the inference of leaping behavior. The overall locomotor repertoire is not unlike that of the forest-dwelling extant saki monkeyPithecia . Considered together, the mosaic of dietary and locomotor morphology inHomunculus suggests thatHomunculus inhabited an environment – as compared with earlier Colhuehuapian and Pinturan primate habitats – shifting towards greater seasonality in patchy forests near river courses.

Organic light-emitting devices (OLEDs) emitting near-infrared (NIR) light have many potential applications, yet the efficiency of these devices remains very low, typically ˜0.1% or less. Here we report efficiency NIR OLEDs based on two fluorescent donor-acceptor-donor oligomers and a phosphorescent Pt-containing organometallic complex. External quantum efficiencies in the range of 0.5–3.8% with emission peak ranging from 700 to 890 nm have been achieved.
